K record sheet for assessment - reports REVISED 2016.doc

  • We use this form at each of the 3 reporting periods. 1st term - pencil, 2nd - blue pen, 3rd - red pen. All items are then on one sheet and are continuous, We can revisit areas of concern.

  • These are documents we are using to assist with the "recording of results" for the Nanaimo Ladysmith Literacy Assessment. They are all drafts at this point that we are tweaking as we see the need.

  • This record sheet for Kindergarten parallels the Early Numeracy Assessment. We are currently working on a "protocol" sheet to go with this record sheet.

  • I use this grid starting in January to really do some criteria referenced assessment of Journal entries that have followed "Writer's Workshop" sessions.
